Some woes with the Manage Update Sites dialog

Description
The essence of http://fiji.sc/bugzilla/show_bug.cgi?id=635 are three issues with the Manage Update Sites dialog:
- when adding a site, the name should be editable before a valid URL is provided
- non-standard update sites should be retained even if they are disabled
- whenever a URL is typed in manually, it should not complain 'Not a valid URL!'
While at it, add the button to the easy mode (many more people will now start to want to follow update sites).
